Submit a request for a veterinary or rehabilitation appointment
Before you Request an Appointment
To help us prepare for your pet's visit and avoid delays, please follow the steps below when requesting an appointment.

NEW CLIENTS : Complete your intake form
New clients must complete our intake form before their appointment can be officially scheduled. You may submit your appointment request first, but please complete the intake form as soon as possible to avoid scheduling delays.

ALL CLIENTS : Tell us why your pet is coming in
In the Special Requests section of your appointment request, describe your pet's main concern and reason for the visit.
Please note: Online submissions are appointment requests; not confirmed appointments
A member of our team will review your submission and you will receive a confirmation email once it has been officially scheduled.
Appointments that must be scheduled by phone
The following appointments must be booked by calling in:
- Surgery/Dentals
- End of life care
- House call appointments
- Diagnostics
- Injections

Rehab therapies
Appointments for the following services require an initial rehab assessment with our veterinarian before booking with a CCRVT:
- Laser
- Underwater treadmill
- PEMF (Magnawave)
- Acupunture
*If you're interested in these therapies please schedule an initial rehab assessment with the vet first.
Vaccine appointments
If your pet needs vaccines please select exam/consult as the reason for visit.
In the special requests section please indicate which vaccines are needed. If you are unsure please indicate you are unsure of vaccine history.
